Health Information Consent

Rooted asks about your health — how you sleep, how you eat, how you feel, and sometimes about depression, anxiety, self-harm, and trauma. That is sensitive information, so here is plainly what we do with it.

• Rooted is not a healthcare provider. It is a wellness and education tool. Because we are not providing treatment or billing insurance, Rooted is generally not covered by HIPAA. We still protect your information as described in our Privacy Policy, and we hold ourselves to those commitments.
• Your answers are yours. They are stored privately in your account and used to build your plan, recommend services, and show you crisis resources when your answers suggest you may need them.
• We never sell your health information and we do not use it for advertising.
• Sharing is your choice. Nothing goes to a clinician, provider, or anyone else unless you turn sharing on or send it yourself.
• No one is monitoring your answers. Rooted cannot respond to an emergency. If you are in danger, call or text 988, or call 911.
• You can leave at any time. Settings lets you pause your account or delete it permanently, which removes your assessments and answers.
