Terms of Service

Rooted is a wellness and education platform. It helps you understand your needs across several areas of life and points you toward local services, providers, and support that may fit.

Rooted is not medical care. Using Rooted does not create a provider-patient or nurse-patient relationship between you and Grounded Initiative Holdings LLC or anyone working with it. Rooted does not diagnose conditions, provide treatment, prescribe anything, or replace professional judgment.

The questionnaires in the assessment — including the PHQ-9, GAD-7, and PC-PTSD-5 — are screening tools, not diagnostic ones. A score indicates whether it may be worth speaking to a professional. It is not a diagnosis, and only a qualified clinician can make one.

No one monitors your results. Rooted shows crisis resources when your responses suggest you may need support, but that is automated. Nobody is watching, and Rooted cannot respond to an emergency or contact anyone on your behalf.

You must be 18 or older and able to enter a contract. One account per person, and the information you give us should be accurate — the plan Rooted builds is only as good as what you tell it.

Rooted offers:

• Rooted — $7.99 per month, or $69 per year.
• Rooted+ Membership — $14.99 per month, or $149 per year.
• Deep Report — $10, one time, no subscription.
• Provider Pro — $29 per month. Provider Practice — $99 per month.

Subscriptions renew automatically at the price and interval you chose, charged to your payment method, until you cancel. Monthly plans renew every month; annual plans renew every year.

You can cancel at any time from Settings, which opens the billing portal. Cancelling stops future charges and keeps your access until the end of the period you already paid for. Cancelling is not harder than signing up.

Refunds. Contact us at info@groundinitiativeproject.com and we will work it out with you.

If we change prices, we will tell you before the change applies to your renewal.

The catalog lists third parties — clinicians, businesses, instructors, and community organizations. They are not employed by or partnered with Grounded Initiative Holdings LLC unless we say so, and a listing is not an endorsement or a guarantee of quality, licensure, availability, or price.

When you engage a provider you are contracting with them, not with us. Payment, care, scheduling, and any dispute are between you and them. We encourage you to verify a provider's license and credentials yourself.

Prices shown are estimates supplied by providers or gathered from public sources, and may be out of date.

What you enter stays yours. You give us permission to use it to operate Rooted for you, as described in the Privacy Policy. Reviews and other content you post publicly should be honest and your own.

Don't use Rooted to break the law, impersonate someone, post false provider information or fake reviews, scrape or resell the catalog, interfere with the service, or try to reach data that isn't yours.

Rooted's software, design, written content, and brand belong to Grounded Initiative Holdings LLC. The screening instruments are the work of their respective authors and are used under their terms. You may use Rooted for your own personal wellness — not to build a competing product.

Rooted is provided as-is. We do not warrant that recommendations will fit your situation, that listed providers will be available or suitable, or that the service will be uninterrupted or error-free.

To the fullest extent the law allows, Grounded Initiative Holdings LLC is not liable for indirect or consequential damages, and our total liability is limited to what you paid us in the twelve months before the claim. Some jurisdictions do not allow these limits, in which case they apply only as far as the law permits.

Nothing here limits our liability for fraud, or for anything the law says cannot be limited.

You can stop using Rooted and delete your account at any time. We may suspend or close an account that breaks these terms or that puts other users at risk. If we close your account without cause, we will refund the unused part of what you paid.

These terms are governed by the laws of the District of Columbia, without regard to conflict-of-law rules. Disputes go to the courts of the District of Columbia.

If we change these terms in a way that matters, we will tell you before the change takes effect.
